Form Guard is a SAAS (software as a service) company that helps ensure medical practices are faster and more efficient for patients to transfer data from any web browser on any device!

Our Approach

In 2009, we observed an increasing trend of medical practices wanting to provide a way for patients to complete paperwork remotely and in advance of the patients appointment. However, practices were forced to only allow patients to download the documents from the practice website, then require patients to print these documents and complete before attending a scheduled appointment. This created a lot unnecessary steps for the patient and wasn't a benefit to etiher the patient or the practice. Having had first-hand experience of this, we designed Form Guard to be patient-friendly, business-focused and flexible enough to customize to fit your medical practice's spefiic needs while you progress through various stages of growth.
